    Our response:

    Now I can see photos and X-ray – This is indeed complex case with aligners only
    Need to assess with cbct scan to start with…What makes it extremely difficult is:

    1 angulation of uneruoted 23
    2 posterior right cross bite
    3 deep curve of spee

    Suggest:
    Assuming all OK on cbct you need to determine with software path of least resistance
    If you recall my lecture on S.A.C protocol, This is complex in micro macro

    Patient factors you need to assess

    I also suggest Invisalign only and warn the patient it may take 2-3 years

    Hi George,

    I was wondering if this is a good case to be done with aligners? My concerns is the buccal corridors. I am wondering if she needs a Maxillary expander before aligner therapy or if aligner therapy is sufficient to move the posterior teeth to fill that buccal corridor space.
